入職要求：

  • 註冊社工，持有認可之社會工作文憑 / 副學士或以上學歷；香港中學文憑考試或香港中學會考中國語文、英國語文考獲第 2 級 / E 級或具備同等學歷；對長者服務有熱誠；具良好溝通能力、有耐性、具責任感、能獨立工作；具長者服務或認知障礙症服務經驗者優先考慮。

職責：

  • 與跨專業團隊合作﹑為區內長者進行個案管理及制定個人照顧計劃；籌備及推行個案及護老者活動；協助中心日常運作、行政及個案輔導工作。
